  Activity ChecklistThis is more that an activity checklist although it includes a check list that you are familiar with, it also allows you to send a picture and other insight into your wants and needs. When you are finished, you can send your submisson to Sir Kelly or the Dom/me of your choice, and as your experience grows you can modify the form and of course send it again to any Dom/me you like. If there are questions that you don't want printed in your report, just leave them blank and they won't be included in your report at all. |
